Vehicle Release Form

I hereby authorize the release of the below listed vehicle to Collision Care Pros, LLC. and their representatives, to take possession of the vehicle, the keys and contents. My signature also releases Collision Care Pros, LLC. from any liability for the vehicle or its contents.

My signature further confirms that I have removed all person possessions from the vehicle and the vehicle will be left in the premise and our insurance is not liable for lost, stole or any damages until I pick up. All insurance coverage between the renter and your insurance company, in the event the vehicle becomes a "Total Loss" and a lien is placed it is then between Collision Care Pros, LLC. and the finance company to settle this matter to further resolve the unpaid balance for repairs, storage or legal fees.
